About The Exam

What is Next-Gen NCLEX®?

It is a new version of the NCLEX that is designed to better assess clinical judgment in nursing candidates.

NGN-style items have two scoring methods to allow for partial credit.
All of the new items are either correct or incorrect. Some will have partial credit. This is called polytomous scoring.

Next-Gen NCLEX® Case Studies

Questions?

FAQ's

NGN case studies, also known as unfolding case studies, present a clinical scenario with client data that changes over time. You will need to answer six NGN test questions. Each question measures one of the six clinical judgment cognitive skills (recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, and evaluate outcomes). NGN Case Studies require that entry-level nurses make multiple clinical decisions throughout the spectrum of the clinical judgment model.

NGN stand-alone items are individual questions that are not part of the case study but have a similar format. There are two types of stand-alone questions:

Bow tie Item — presents a clinical situation at one point in time and can measure all six clinical judgment cognitive skills in that one question.

Trend Item — presents a clinical scenario in which a student reviews the information in the medical record that trends (happens) over time (minutes, hours, days, or more). Trend Items address one or all of the six clinical judgment cognitive skills.

Each case study includes a client scenario and follows Layer 3 of the NCSBN Clinical Judgment Measurement Model (NCJMM) in sequential order, focused on each step from “Recognize Cues” through “Evaluate Outcomes”.

Each item within a case study is independent and scored accordingly. Candidates will not be able to go back to view previous responses and care is taken to avoid cuing within each case study.

More information about how NGN items are scored can be found in this article.

Beginning with the Next Generation NCLEX launch, items containing a numeric laboratory value will include the corresponding normal reference range.

● MULTIPLE CHOICE: SINGLE RESPONSE — This item type is similar to the traditional NCLEX format; however the clinical scenario will have more information, and the number of responses could be as many as ten options.

● MULTIPLE RESPONSE (SELECT ALL THAT APPLY) — This item type is similar to the traditional NCLEX format select all that apply questions, but with up to 10 options.

● EXTENDED DRAG AND DROP — This item type is like the traditional
NCLEX-ordered response items, but not all response options may be required to answer the item.

● CLOZE (DROP-DOWN) — Allow candidates to select one option from a drop-down list. There can be more than one drop-down list in a Cloze item.

● ENHANCED HOT SPOT (HIGHLIGHTING) — Allows candidates to select their answer by highlighting pre-defined words or phrases.

● MATRIX/GRID — Allows the candidate to select one or more answer options for each row and/or column.

Yes, some NCLEX Multiple Response Select all that Apply items are scored using the +/- scoring method on the NGN Exam that went live on April 1, 2023.
